Abstract

Cialis and other drugs used to treat erectile dysfunction are widely recognized among the top 10 medications in the United States. Despite their proven efficacy and safety, many patients and healthcare providers are unaware of how these drugs work and what the benefits are. This study describes the pharmacokinetics and clinical implications of cialis in the presence of mild-to-moderate liver impairment, which is one of the main reasons for the low prevalence of the drug in the United States. This study also highlights the importance of pharmacokinetic data to inform the design and analysis of clinical trials.

Methods

We conducted a retrospective observational review of 673 patients treated with cialis (brand name “Tadalafil”) from 2008 to 2012. We analyzed the pharmacokinetic profiles of the drug (compared to placebo) by the following statistical analysis: (i) the percentage of cialis excreted and (ii) the percentage of cialis excreted in urine by the pharmacokinetic analysis. A total of 4,564 patients were included in the analysis. A detailed medical record review identified 792 patients, with the most recent drug therapy recorded in the medical record of all patients (i.e. in the first 6 months of the study period). Patients who received cialis had a mean age of 64.5 years. Of the total number of patients (mean age = 67.5 ± 5.4) with liver disease, the mean age of the drug-treated patients was 60.4 years (mean ± standard deviation = 58.5 ± 4.2). In the first 6 months of the study period, the mean age of the patients with liver disease was 60.9 years (mean ± standard deviation = 59.8 ± 4.1). A total of 5,069 patients (mean age = 62.6 ± 3.3) had liver disease. The mean age of patients with liver disease was 67.8 years (mean ± standard deviation = 63.2 ± 4.4).
